Physical Dates 2022-2023

Physicals are required prior to the first day of mandatory practice in any sport a student participates in during the 2022-2023 school year. 

Physicals must be completed after June 1, 2022 in order to be valid for the 2022-2023 school year.

********Athletes are required to have a completed pre-participation packet in order to get a physical.  These can be attained at the following link    https://app.formreleaf.com/organizations/burgettstown-jr-sr-high-school .  Please create an account in the upper right hand corner before registering your child for the appropriate sport.  The Pre-Participation packet is under each sport listed and must be filled out before the physical is given.

Physicals will be given for athletics on the following dates in the High School Nurses Office:

Winter Sports Physicals

 Winter sports physical will be given in the nurse's office on the following dates....

•  9/28/2022       Wednesday    12:00 PM - 2:30 PM
•  10/5/2022       Wednesday      9:00 AM - 12:0 PM 
•  10/13/2022     Thursday          9:00 AM - 12:00 PM

All athletes will need a completed physical packet.  Packets can be found at the link   https://app.formreleaf.com/organizations/burgettstown-jr-sr-high-school .  

Any athlete that participated in a fall sport will need a Section 8 (used to be section 7).  They can be found in the main office.  They are yellow.
